Issue of Duplicate Certificate

A member can apply for issuance of duplicate membership certificate or Certificate of Practice in the following circumstances:-

a.         The original certificate/s have been lost.

b.         The original certificate/s in possession are mutilated.

c.         The original certificate/s have been damaged.

 

Requirements:

a.            A written request for issuance of duplicate certificate/s.

b.         An affidavit on non-judicial stamp paper of Rs.20/- in the format available on the website affirming the specific reason and duly attested by Notary Public / Oath Commissioner /First Class Magistrate.

c.       The duplicate certificate charges are Rs.236/- (including GST) per certificate. The payment may be made by cheque at par/demand draft payable at New Delhi favouring “The Institute of Company Secretaries of India”

d.         In case of damaged certificate(s), the member should return the same along with the application and duplicate certificate fee.

 

Note: The affidavit is not required to be executed in case the original mutilated or damaged certificate is returned to the Institute.
