A member can apply for issuance of duplicate of the original membership certificate or Certificate of Practice in the following circumstances:-
  • The original certificate/s have been lost.
  • The original certificate/s in possession are mutilated.
  • The original certificate/s have been damaged. 
Requirements:
  • A written request for issuance of duplicate certificate/s.
  • An affidavit on non-judicial stamp paper of " Rs. 10/- in the format available on the web-site affirming the specific reason and attested by a First Class Magistrate/Oath Commissioner/ Notary Public.
  • The duplicate certificate charges are Rs. 20/- per certificate.  The charges may be remitted by a local cheque or by way of demand draft, in favour of ‘The Institute of Company Secretaries of India’, payable at New Delhi.
  •  In case of damaged certificate(s), a member should return the same alongwith the application and duplicate certificate fee.
An affidavit is not required to be executed in case the original mutilated or damaged certificate is returned to the Institute.
