PROCEDURE FOR EFFECTING CHANGE OF NAME IN THE INSTITUTE’S RECORDS
The members may get their name changed in the Institute’s record by submitting the following documents:
1. In case of Female members on their marriage - Application for change in surname along with any one of the following documents:
a) Marriage invitation card in original
b) Copy of Marriage Registration Certificate duly attested by a Gazetted officer/Member of the Institute
c) An affidavit (in original) on non-judicial stamp paper of Rs.10/- affirming the date of marriage with details duly attested by First Class Magistrate/Notary Public/Oath Commissioner as the case may be
2. In all other cases:
Application for change in name alongwith Extract of Gazette notification duly attested by a Gazetted officer/Member of the Institute and original advertisement as published in a leading newspaper.
